Free Pediatric Visit Request

At Levine Chiropractic we're passionate about nurturing healthy families, and that starts with our children! That's why we're offering complementary initial chiropractic visits for kids throughout the month of May. This offer is available for all kids, from newborns to 18 years old. 

Use this form to request an appointment with us. When requesting a free initial visit for Kids' Month, make sure to select "Free Initial Kids Visit" under the "Patient Type" section. While we will do our best to accommodate your requested day and time, please note, your appointment is not fully booked until you get a confirmation from us! 

Please note that, per federal and state law, this offer is not applicable with any federal or state funded insurance programs. Should you have Medicare or Medicaid insurance, please call our office to discuss your care options (we are in network with most Medicare and Medicaid carriers).
