Your First Visit

Patient Forms

Upon entering our office, our receptionist will welcome you and get you started! We will have you complete your new patient paperwork, providing us with your health history and information on your condition (you may also complete your new patient paperwork ahead of time here). Please make sure to bring your ID and your insurance card, if you will be using health insurance. 


Consultation

Next, you will have a consultation with one of our Doctors of Chiropractic to discuss your health-related problems, concerns, occupational history, and other factors which may be contributing to your symptoms. This initial visit is designed our doctors to learn more about you, your condition, and start to form a plan of action for your care. 


Examination

After your consultation, your chiropractor will perform a complete chiropractic examination, consisting of orthopedic, neurological, and range of motion tests. This allows us to best understand what is happening in your spine, to learn where abnormality may exist and how we can correct it. However, nothing will be done in our office without your consent. 

Chiropractic is not about treating symptoms, but addressing the root cause of your problem. Your examination is the first step in determining that underlying cause. 


X-Ray Studies

Depending on your specific condition, we may take X-rays as well. We have an advanced digital X-ray suite right in our clinic that allows us immediate results. 

X-rays help us determine what and where your exact issue is and how we need to address it. Imaging also allows us to see if there is anything else going on in your spine that your primary care doctor or any other specialist needs to be aware of. Most people are amazed once they see their X-rays up close.


Report Of Findings

Once the information is collected and examinations are performed, your Doctor of Chiropractic will give you a detailed report of your examination and imaging findings. Here we will answer any questions including:

  1. How can you help me?
  2. How often do I need to come in?
  3. What will my treatment cost?

After reviewing your health history, goals, and examining your spine and X-rays, Robert Levine will discuss recommendations and notify you if your condition requires care with other providers. We will provide the best treatment and wellness program for your needs.


Treatment

Once you have been examined, we've analyzed your images, and determined which style of care will be most appropriate and effective for your condition, you can experience your first adjustment. 

Wellness Program

Prior to leaving, the doctor will suggest a wellness program to incorporate outside of treatment. If you are in pain when you first come into our office this may include: ice or heat application instructions, certain activities or positions to avoid, and at home exercises and/or stretches. If you desire, our wellness team will work with you also to create healthy habits and routines for your lifestyle. Every person is unique, therefore everyone requires a customized wellness plan. The purpose of our wellness program is for you to achieve good spinal alignment, have a healthy diet, exercise, and maintain a positive mental state.
