As our way of saying thank you, Levine Chiropractic is offering a complimentary initial chiropractic evaluation to anyone in your household. This includes a chiropractic consultation, examination, and any necessary imaging.

All additional visits will be the patient’s responsibility, either through your health insurance carrier or as private pay. Please note that if your care is not considered active care under your insurance carrier’s guidelines, we are unable to bill your insurance for those services.

In compliance with federal and state law, this offer is not applicable for patients with federal or state-funded insurance programs (such as Medicare or Medicaid).

To qualify for this promotion, family members must reside in the same household. This offer is only available for new patients.
